第一版前言(Foreword to the First Edition)任何臨床醫師都不願意在尚未詢問與瞭解病人的病史之前，且尚未進行相關基本理學檢查技巧的情況下，貿然地進行臨床處置。同樣地，也不會在對藥物的使用方式沒有基本了解的情況下，給予病人臨床治療。過去的臨床醫師會感覺自己是處在尚未知如何去具體解釋病人的病史與理學發現，或尚未知如何去預期藥物療效與反應強度的情況下，即給予病人藥物治療。實證醫學(EBM)為此問題提供了解決方式，於1970年代，開啟了教育臨床醫師有效地使用醫學文獻的風氣，並在1980年代演變為將其應用到整個臨床照護系統。這個新的方式非常受歡迎，在1991年的醫學文獻中，命名為「實證醫學(evidence-based medicine; EBM)」[1]。在隨後的16年中，實證醫學的演進與發展，不僅代表如何有效的運用醫學文獻，同時也是臨床決策過程中重要的方向指導原則。一般民眾對於大多數醫師仍然無法嚴格的評讀研究文章與掌握研究結果中所隱含的所有內容而感到震驚。相同地，對於醫師而言，缺乏評讀研究的能力，限制了醫師的獨立性；結果是只能依賴專家的意見、同事及來自藥廠的資訊。但如果專家與同事的意見並不一致，或者對藥廠提供的建議有所質疑時，甚麼才是合適的臨床處置呢？這本書提供了一把開啟評讀醫學文獻大門的鑰匙給臨床醫師。打開這個禁地的大門之後，增強的能力是巨大且有力量的，醫師們再也不必因為建議是來自於權威性或聲量最大的專家，而採取選擇相信。臨床醫師可以具有區分高品質證據和低品質證據的能力，也能獨立判斷甚麼樣的治療對病人是最好的。臨床醫師也有能力對病人本身解釋替代方案的影響，確保病人的選擇與病人的基本價值觀和偏好是一致的。約十年前，專家及其學會都一致推薦停經後的女性長期使用荷爾蒙替代療法(hormone replacement therapy; HRT)。這些建議大部分是立足於觀察性研究的基礎上所提出來的，建議婦女服用荷爾蒙替代療法(HRT)，降低其發生重大心血管事件的風險。實證醫學的支持者對此類僅由薄弱的觀察研究方法，即對如此龐大的婦女做出強烈的建議表示擔憂，但大多數他們的聲音是被忽略的，直到隨機試驗的出現，證實之前觀察研究的結果是不正確的，HRT對心血管疾病的影響就是增加心血管疾病的發生頻率。現在，許多臨床學會組織都贊同廣泛的篩檢，可有效預防癌症和心血管疾病的
