言語－語言障礙評估：臨床篩檢與檢查

 根據劍橋字典 (Cambridge Dictionary)，語言係指一連串聲音（頻率）所構成的訊息，經由接收者的大腦轉譯為有意義的指令或資料。語言可以分成兩種，分別為人類自然發展而來的自然語言 (Natural Language)，以及人類因需要開發指令或其他需要而創造的人工語言 (Constructed Language)。本書主要著重於自然語言。

語言是人類生活中人際交往的工具，不僅可以用來傳達意見和表示感情，也能夠作為適應環境及解決問題時幫助思考的工具。典型的一般人（含兒童、青少年和成人）通常在自然的環境中就可以順利地獲得、產生和理解聲音、單詞、句子和慣例的規則，以便在各種社會情境中適當地使用它們。不過有些人卻會由於生物的成因（有關中樞神經系統或身體內其他系統的功能及結構問題），或者是環境的成因（如貧窮、嚴重的過敏症、耳朵重複發炎干擾到聽力、濫用聲音或強調產生說話所涉及的肌肉、忽視或虐待），而形成異常或障礙。為求有效的治療或介入，首要的任務就是實施診斷和評估，試著找出可能造成言語–語言遲緩、異常或障礙的原因，然後再計畫地進行語言治療介入，以求恢復正常的言語–語言功能。

是故，筆者們特別搜羅國內外有關言語–語言障礙評估的相關文章、叢書和期刊論文，撰寫《言語–語言障礙評估：臨床篩檢與檢查》專書，藉以提供職前語言治療師培育、現場語言治療師、一般學生、家長和社會人士能夠對於言語–語言障礙評估有個概括性的認識和理解。

基本上，全書分成「言語–語言障礙評估的基礎」和「異常∕障礙類型之言語–語言障礙評估」等兩大類，共計分為 11 章來探究。其中「言語–語言障礙評估的基礎」包含：第 1 章緒論（張世彗）；第 2 章要探討語言治療師服務傳遞的諮商（張世彗）；第 3 章則簡要敘述言語與語言評估之心理計量分析（張世彗）。「異常∕障礙類型之言語–語言障礙評估」則包括：第 4 章言語相關之口腔顏面部評估（陳雅資、張世彗）、第 5 章兒童及成人聽力篩檢（鄧菊秀、劉樹玉）、第 6 章兒童語言障礙之評估（邱怡婷）、第 7 章構音和音韻異常之評估（張世彗）、第 8 章自閉症類群障礙之言語和語言評估（張世彗）、第 9 章吞嚥障礙的評估（巴甫仁、吳家欣）、第 10 章運動言語障礙之評估（陳雅資），以及第 11 章語暢異常之評估（楊淑蘭）等。

本書得以順利付梓，要特別感謝合記圖書出版社的大力地支持。雖然筆者們皆已勉力撰寫本書，惟因才智有限，謬誤和缺失之處，恐無法避免，尚祈方家不吝評論和指教。

Given the eclectic nature of Communication Sciences and Disorders, an introductory textbook on assessment principles and implementation is not an easy undertaking. Simply introducing important concepts and methods behind the development of assessment tools is easily a book in itself. Add to that differences in approaches because of age (pediatric, adult or geriatric), setting (hospital, private practice, school or community), and function (physical, cognitive, communicative, or socioemotional), and the endeavor takes on gargantuan proportions. This book manages to do so by providing a relevant sample of key background concepts and usable tools that therapists and student clinicians can readily implement.

It is largely thanks to the expertise and breadth of knowledge of Professor Chang （張世彗）that much of this book was made possible. Her skillful adaptation of crucial information on diagnostic theory, psychometrics, and speech and language analyses provides the foundation for the remaining chapters in the book. By the same token, Professor Chen（陳雅資）was both the glue and catalyst to this endeavor – her handiwork helped mold the chapters of this book and bring it to fruition. It goes without saying that the expertise of the other authors was a real boon in helping to distill key information from their respective specialties.

Speech-language pathologists are clinicians, not technicians. This means that assessment does not have a simple step-by-step instruction book that always gets you your final product. This may be more obvious with certain specialties (like swallowing or motor speech), but it holds true for all areas in the discipline. Assessment requires a critical mindset and systematic approach to problem-solving. This book should provide the reader with the necessary ingredients for carrying out well-rounded assessments that benefit both the patient and the profession.
